Appointment request privacy information

The appointment request form collects only what reception needs to arrange your visit: the patient's name, a contact phone number, an optional email address, the speciality or doctor you prefer, and your preferred date and time.

The form does not ask for medical history, reports, identity documents or payment details. Please do not include them.

Your request is visible only to authorised hospital reception and administration staff, who use it to contact you about this appointment.
